DRIVING APPROVALS

If you will be driving a company vehicle during your time here, you will need to fill out a Company Vehicle Usage Agreement and pass it to a member of the Company Management team. If you have never completed this form before, please also complete a General Consent Form that will allow us to pass your information along to our insurance agency for approval. Please note that this form needs to be signed in front of a Notary Public.

Everyone who plans to be driving a vehicle for Glimmerglass this year will need to fill out this form. You may not get behind the wheel of a car for the company until you have been informed of your approval. Each driver for the company will need to go through the approval process once a year and will need to participate in vehicle training during orientation once every three years.
